    Spontaneous lung herniation is a rare phenomenon in which the lung parenchyma along with the pleural membranes protrudes outside their usual boundaries and can lead to a wide variety of complications. We are reporting a case of a middle-aged male who presented with chronic obstructive pulmonary disease (COPD) exacerbation with severe bouts of cough. Initial computed tomography (CT) chest was unrevealing, but two days later, he developed spontaneous lung herniation, which was initially managed conservatively, but later it progressed to pneumothorax, pneumomediastinum, with striking CT scan images showing extensive subcutaneous emphysema. Blowhole incisions were done on the anterior chest wall which led to ultimate recovery

    Posterior reversible encephalopathy syndrome (PRES) is an increasingly recognizable neuro-clinical syndrome. Clinical and neurological manifestations of PRES include hypertension, headache, encephalopathy, seizures, and symmetrical white matter changes on brain MRI. Most common precipitants of PRES are acute medical illness, hypertensive crisis, eclampsia, immunosuppressive therapy, and chemotherapy. Bevacizumab is a monoclonal antibody that halts angiogenesis by inhibiting vascular endothelial growth factor. It has gained widespread popularity in oncology world especially for metastatic and recurrent cancers due to its inherent ability to stop angiogenesis; a vital step for tumor growth. Bevacizumab has also been implicated as the cause of PRES due to dysregulation of the blood-brain barrier. We are reporting a case of PRES induced by Bevacizumab in a patient of colorectal cancer

    Knowledge and practices among male medical students regarding smoking and its hazards

    Background: Tobacco use is the leading preventable cause of death worldwide, and according to the latest estimates, by year 2030, 10 million annual tobacco-attributable deaths worldwide, 70% of which will be in low-middle-income countries. The purpose of study was too assess the knowledge and practices regarding cigarette smoking among male medical students in Allama Iqbal Medical College, Lahore, Pakistan and to assess the awareness of students regarding smoking related health hazards.Methods: A Cross-Sectional Study was conducted From May 2009 - September 2009 in Allama Iqbal Medical College, Lahore, Pakistan. The male students enrolled in Allama Iqbal Medical College Lahore, were selected by non-probability convenient sampling.Results: In present study 47.3% respondents were smokers, 94.1% were knowledgeable about hazards of smoking, 85.9% were aware about passive smoking, and 19.1% started consuming tobacco below 17 years of age. Only 19.1% tried to quit smoking but none of them succeeded. 25% have no family association of smoking. There were 18.2% who’s mostly close friends were smokers only 13.2% smokes more than 15 cigarettes daily. 43.6% said they would smoke cigarette if one of their close friends offer them. 30.5% believe cigarette smoking help to relax and 25% want to quit smoking among them 10% want to quit due to its hazards while 5.9% due to family pressure.Conclusions: Present study concluded that high proportion male medical students smoke cigarette. Most of them were unaware about tobacco consumption hazards, and passive smoking.

    MaintainoMATE: A GitHub App for Intelligent Automation of Maintenance Activities

    Software development projects rely on issue tracking systems at the core of tracking maintenance tasks such as bug reports, and enhancement requests. Incoming issue-reports on these issue tracking systems must be managed in an effective manner. First, they must be labelled and then assigned to a particular developer with relevant expertise. This handling of issue-reports is critical and requires thorough scanning of the text entered in an issue-report making it a labor-intensive task. In this paper, we present a unified framework called MaintainoMATE, which is capable of automatically categorizing the issue-reports in their respective category and further assigning the issue-reports to a developer with relevant expertise. We use the Bidirectional Encoder Representations from Transformers (BERT), as an underlying model for MaintainoMATE to learn the contextual information for automatic issue-report labeling and assignment tasks. We deploy the framework used in this work as a GitHub application. We empirically evaluate our approach on GitHub issue-reports to show its capability of assigning labels to the issue-reports. We were able to achieve an F1-score close to 80\%, which is comparable to existing state-of-the-art results. Similarly, our initial evaluations show that we can assign relevant developers to the issue-reports with an F1 score of 54\%, which is a significant improvement over existing approaches. Our initial findings suggest that MaintainoMATE has the potential of improving software quality and reducing maintenance costs by accurately automating activities involved in the maintenance processes. Our future work would be directed towards improving the issue-assignment module

    Degradation of Guar-Based Fracturing Gels: A Study of Oxidative and Enzymatic Breakers

    Unbroken gel and residue from guar-based fracturing gels can be a cause for formation damage. The effectiveness of a fracturing treatment depends on better achieveing desired fracture geometry, proper proppant placement and after that, a good clean-up. The clean-up is achieved by reducing the fluid viscosity using chemical additives called "Breakers". There are many different types of breakers used in the industry, but they can be broadly divided into two categories: oxidizers and enzymes. Breaker perfromance depends on bottomhole temperature, breaker concentration and polymer loading. Different kind of breakers, used at different concentrations and temperatures, give different kind of "break" results. Therefore, the amount of unbroken gel and residue generated is also different. This project was aimed at studying basic guar-breaker interactions using some of the most common breakers used in the industry. The breakers studied cover a working temperature range of 75 degrees F to 300 degrees F. The effectiveness of each breaker was studied and also the amount of damage that it causes. Viscosity profiles were developed for various field concentrations of breakers. The concentrations were tested over temperature ranges corresponding to the temperatures at which each breaker is used in the field. The majority of these viscosity tests were 6 hours long, with a few exceptions. Early time viscosity data, for the intial 10 minutes of the test, was also plotted from these tests for fracturing applications where the breaker is required to degrade the fluid by the time it reached downhole. This was needed to prevent the damage to the pumping equipment at the surface yet still have almost water-like fluid entering into the formation. The study provides a better understanding of different breaker systems, which can be used in the industry, while designing fracturing fluid systems in order to optimize the breaker performance and achieve a better, cleaner break to minimize the formation damage caused by polymer degradation

Being more convenient online shopping seems painless compared to in store shopping, why consumers are still reluctant to shop online?.A sample of 220 questionnaires was filled from different departments at University of Sargodha. Respondents were asked to fill the questionnaire based on four factors (Perceived cost, Perceived risk, Perceived convenience, Perceived enjoyment), there were 2 general questions. Out of 220, 207 questionnaires were returned. One sample test is applied in this study to check the reliability of independent variables. Analysis was also done on basis of gender and their ages. The most important factor out of four, which can persuade the customer's online buying decision, is delivery cost for purchased items and it has negative relationship with dependent variable, moreover perceived risk has also negative relationship with dependent one. Perceived convenience and Perceived enjoyment has positive relationship with online shopping preference. As online shopping is easier to do but due to extra delivery cost and risk factors consumers do not adopt online shopping and these factors should be minimized to promote online shopping. Online shopping should be promoted and to gain the consumer confidence, delivery cost and risk factor should be minimized. As online shopping is easier to do as compared to offline shopping but people still reluctant to use internet for online shopping, so to promote online shopping delivery cost and risk factor should be minimized to gain consumers attraction and confidence. People want to experience online shopping but it won’t be promoted until delivery cost and perceived risk factors be reduced. A little work has been done on exploring the factors that influence the online buying decision. These factors are called situational factors and include delivery charges, risk factors, convenience factors and enjoyment factors. In Pakistan consumers are not too much affiliated to online shopping as compared to European countries and not too much researches have been done related to online shopping in Pakistan , so this research been conducted to promote online shopping

    Role of Gum Chewing After Small Gut Anastomosis

    This study was performed to access the effectiveness of gum chewing on gut motility in early postoperative period after small gut anastomosis in patients operated for reversal of ileostomy or ileocolostomy. Methods: This prospective, randomized trial was undertaken at Surgical Unit-I, Holy Family Hospital, Rawalpindi Medical University, Rawalpindi from March 2016 to March 2018. 90 patients were randomly divided into two equal groups. All patients underwent small gut anastomosis after reversal of ileostomy or ileocolostomy. In group A (gum chewing group), patients were advised to chew gum, whereas group B (control group) patients did not received any gum for chewing and continued standard post-operative care. Both groups were kept NPO till 2nd post-operative day. After surgery, all patients were observed for onset of bowel sounds, time after surgery when flatus and stool were passed, time required for return of appetite and discharge from hospital. Patient satisfaction with postoperative course, incidence of anastomotic leak, wound infection and mortality was also recorded in both groups. Results : Statistically, group A and group B were similar in age, gender and type of procedure. There was significant difference between both groups for onset of bowel sounds, passage of first flatus, complaints of nausea and vomiting, appetite, duration of hospital stay and patient’s satisfaction level (P < 0.05). In Group A, 35 patients were satisfied whereas in group B, only 5 patients were happy with their postoperative course (P < 0.05). There was no statistically significant difference between groups for incidence of anastomotic leak, wound infection and mortality. Conclusion: It is safe and cost effective to advise gum chewing in patients undergoing small gut anastomosis for reducing duration of postoperative ileus and improving level of patient satisfaction about their postoperative course.     A prospective comparative study of Proximal Femoral Nailing Anti-rotation (PFNA) and Sliding Hip Screw (SHS) for Per-trochanteric Femur Fracture

    Objectives: To determine the functional outcome, operative risks, rate of union, and complication in Per-trochanteric  Fracture fixed with PFNA and SHS. Material and Methods: The present study has been conducted at Shalamar Medical and Dental College Lahore from January 2018 to December 2018.40 patients with per-trochanteric femur fracture treated with proximal femoral nailing anti-rotation (PFNA)  and Sliding hip screw (SHS) were enrolled in our study.20 patients were treated by PFNA and 20 patients by SHS. Timing of surgery, mobilization status, hospital stay, infection, weight-bearing status, radiological union, complications both technical and implant-related, amount of blood loss(ml), C ARM Exposures, and Harris hip score at the end of 6 months were recorded. Results: Union was better in the PFNA group (95%) as compared to the SHS group (85%). Complication rate, hospital stay, surgery timing, and requirement of revision surgery were more in the SHS group. The functional outcome was better in the PFNA group as compared to the SHS group. Conclusion: From our study, we concluded that PFNA is a better alternative than SHS in terms of higher union rates, low complication rates, and better functional outcomes. &nbsp
